    • the Applicant has read the Program Guidelines and fully understands the content;
    • the information provided by the Applicant in this application and the exhibits are true, correct and complete; and
    • the Applicant understands that the applications for the grant program are being evaluated on a first come, first reviewed basis and that there can be no more than one application accepted per storefront.

    • Evidence of business ownership (e.g., the lease if applicant is a tenant; a grant deed or property tax invoice if applicant is the real property owner)
    • Color photographs of the storefront and all sides of building visible from a public right of way
